Thank you for registering for Winchester Pop Warner!

Bear in mind that a complete registration package includes all of the following:

Individual Forms
Player Contract & Consent Form
Doctor’s Medical Form (must be completed by physician in 2012)
Parent Medical Form
Parent Code of Conduct must be completed and signed by ALL parents/guardians)

Other Requirements
- Birth Certificate: This must be an original with a raised seal. WPW holds birth certificates (according to Pop Warner guidelines) for the duration of the season.

Birth Certificates will be returned to parents if requested when equipment/ uniforms have been returned to WPW.  If you did not request that your birth certificate be returned at the end of last season, WPW has kept it on file and you do not need to resubmit it again this year.

- Report Card (both front and back are required). We cannot accept Report Cards until the end of the current school year.  Please do not submit partial year report cards.

- Registration Fee ($295 for football, $275 for Cheer): Credit Cards or Checks are accepted. You may pay your fee via credit card during the online registration process, or pay by check.  Note: Financial need based scholarships are available.  Email mbongiorno@winchesterpopwarner.com for information.
